정관수술 후 아내가 임신했다?! 너무 당황하지 마세요.
“환자분, 왜 갑자기!! (^o^)”
“거기가 가렵고, 뭐가 좀 나서요. ᅲ.ᅲ”
“저… 여긴 이비인후과인데요.^^;”
자주는 아니고, 정말 간혹 있는 일이지만, 해마다 한두 번은 꼭 겪는 일이 이비인후과를
비뇨기과로 착각하고 찾아오시는 환자들이다. 누구한테는 상식인 일이 다른 누구에게는 낯설기만 한 게 비단
이뿐이랴. 이제는 이런 일도 이골이 나서, 환자분이 겸연쩍지
않게 옆 비뇨기과로 안내하는 요령까지 숙달했다. 이 정도는 돼야, 진정
이비인후과 전문의라 할 수 있지 않을는지.
그런데 바로 연달아, 아랫도리(?) 사연이 이번에는 내 메일함으로 날아들었다.
약간은 각색을
했지만, 대략 이런 사연이다. 사실 이비인후과 의사에게 이런
질문이란, 자장면 맛이 일품인 중식당 주방장에게 찾아가, 시원한
열무 국수 마는 법을 묻는 것과 비슷하다. 그런데 정말 가끔은 중식당 주방장이 “자장면도 좀 더 늘리면 국수가 되지 말입니다.” 하며, 마치 기다렸다는 듯이 열무 국수의 비법을 알려 줄 수도 있겠다. 오늘
내 답변도 그쯤이라 생각하고 들어보자.
1. 꺼진 고추(?)도 다시 보자.
정관수술 후 부부관계를 갖기 전에 무정자 상태를 확인받았는지가 우선 궁금하다. 정자는 고환에서 만들어지고, 사정 시에 정관을 지나 요도를 통해 바깥으로 분출된다.
정관수술이란
그 정관을 잘라서 정자의 이동 경로를 차단하는 방법인데, 수술만으로 바로 씨 없는 수박(?)이 되는 건 아니다. 이미 만들어져 정낭과 정관 속에 남아 있던
정자들은 수술 후 2개월에서 길게는 3개월까지(또는 15~20회 정도의 사정 사이클이 돌아가는 동안)도 정액 속에서 계속 관찰된다. 그래서 대부분 병원에서는 수술 후 6주 정도에 병원에 다시 내원해서 정액검사를 통해 무정자증을 확인받길 권한다.
문제는 귀찮다는 이유로 상당수의 아저씨가 이 과정을 셀프로 생략한다는 거고.

정자로 인한 임신 가능성도 충분하겠다.
2. 세상에 100%란 배터리 충전에서나 가능한 일이다.
물론, 무정자증까지 확인받은 경우일 수도 있겠다. 하지만, 그렇다고 해도 정관수술 후 피임 성공률이 100%가 되는 건 아니다. 정관수술 후 피임 실패에 대한 문헌을 찾아보니 아래와 같다. [1]
Intraluminal needle cautery (vas not transected, no segment removed): Less
than 1 percent
Cautery both ends and fascial interruption: 1.2 percent or less
Cautery (prostatic end) only and fascial interruption (clip): 0.02 to 2.4
percent
Cautery of both ends and excision of a segment: 4.8 percent or less
Ligation and fascial interruption: 16.7 percent or less
Ligation and excision of segment: 1.5 to 29 percent
수술 방식에
따라서 적게는 0.02%까지도 보고되지만, 단순히 정관을
자르고 묶는 방법일 경우에는 최대 29%의 피임실패율이 보고되기도 한다. 그러니 정관수술 후 임신이 아주 불가능한 건 아니라는 얘기다.

명심하자.
